Can customers fill in a patch test, consent form or questionnaire when they book?

Yes - under Your shop, Forms, add the forms your trade needs from the templates (patch test, tattoo consent, health questionnaire, new patient intake, pet details, licence details, model release, consent to treatment) or write your own; they are asked on your booking page when the customer books, and the answers sit on their profile.

Under Your shop, Forms, your trade's templates are one tap away, and More templates shows the rest. Edit the questions, choose whether a form is asked on the first visit (and again after it lapses), every visit, or only when you ask, and which services it is for. Health and consent forms end with the agreement wording, a retention line and a typed name.

When a customer books one of those services, the form appears on the booking page under Before you come, after they type their email, and is answered before the booking is made. If they booked by phone, or skipped it, their booking page asks for it and the confirmation email says so. Inside the validity window they are not asked twice.

In the diary, each booking's card shows every form done (press it for the answers) or not yet. The customer's profile lists everything they have completed, and each one opens as the answers with the agreement, with a PDF to download. Sensitive answers are never exported any other way. Forms are free on every plan; switch them on or off under Settings, Features.
